Luke Allen Perkins, 22, passed away at 12:50 pm, Thursday, July 19, 2018 in Chicago Cancer Treatment Center of America, after a courageous battle with cancer. Luke was born on April 7, 1996, the son of Tracy Koontz and Kevin Perkins. He is survived by his fiancé, Anahi Jacobo of Bremen; their daughter, Sophia Rey Perkins; mother, Tracy Koontz of Anna, OH; father, Kevin Perkins of Bremen; sister, Chelci Main of Lafayette; maternal grandmother, Becky Koontz of Greenville, OH; paternal grandmother, Cindy Fisher of Bremen and numerous family members and friends. Visitation will be held from 11:00-2:00 pm, Wednesday, July 25, 2018 in Mishler Funeral Home, 2030 St. Rd. 331, Bremen. Funeral services will follow at 2:00 pm in the funeral home with Pastor Wes Lynch officiating. Per his wishes, cremation will follow. In lieu of flowers, memorials are suggested to help the family with expenses.
